E0194 Air-fluidized bed — pressure reducing support surface, Group 3Medicare Air-Fluidized Bed Documentation Requirements (E0194)
Citations verified against the CMS Medicare Coverage Database · Reviewed September 2026
An air-fluidized bed is the last step in the pressure-ulcer support-surface ladder, and Medicare's coverage rule reads like it: every criterion in LCD L33692 has to be met, in order, with a documented month of conservative treatment on a Group 2 surface that did not work.
E0194 joins the Required Prior Authorization List — and the face-to-face and written-order-prior-to-delivery list — on October 28, 2026. Group 2 surfaces (E0193, E0277, E0371–E0373) have required prior authorization since 2019. Below is the requirement set AffirmDME checks on an E0194 case.

Stage 3 or stage 4 pressure ulcer
The record must document a full-thickness (stage 3) or deep tissue destruction (stage 4) pressure ulcer.
LCD L33692 — Group 3 coverage criteriaBedridden or chair-bound, institutionalization risk, and a trained caregiver
The beneficiary is bedridden or chair-bound from severely limited mobility, would require institutionalization without the bed, and has a trained adult caregiver available for daily care, repositioning, and management of the bed system.
LCD L33692 — Group 3 coverage criteriaA month of conservative treatment on a Group 2 surface, without progress
At least one month of a documented program — frequent repositioning, a Group 2 support surface, treatment of infection, nutrition, debridement, moist wound care — that did not progress toward healing. Other equipment must have been considered and ruled out.
LCD L33692 — conservative treatment programPractitioner assessment within the month before starting
The bed must be ordered after a comprehensive assessment and evaluation by the treating practitioner, performed within one month before therapy begins.
LCD L33692 — comprehensive assessment timingDiagnosis codes for the ulcer stage
The ICD-10 codes on the claim must reflect the documented ulcer and stage.
Policy Article A52468 — pressure ulcer diagnosisFace-to-face encounter documented (required from October 28, 2026)
E0194 joins CMS's Required Face-to-Face Encounter and Written Order Prior to Delivery List on October 28, 2026. From that date the chart notes must document an in-person (or permitted telehealth) evaluation for the item, and the completed order must reach you before delivery.
42 CFR 410.38(d)(2) — F2F/WOPD list, effective 10/28/2026Face-to-face within 6 months of the order
The qualifying encounter must have occurred within the 6 months before the order date.
42 CFR 410.38(d)(2)Standard Written Order (SWO) on file
A complete written order must exist before the claim is submitted — patient name, item description, quantity, ordering practitioner, and signature.
42 CFR 410.38(d)Order date and practitioner signature
The SWO must be dated and signed by the treating practitioner. Undated or unsigned orders are treated as no order at all.
42 CFR 410.38(d)Practitioner NPI on the order
The ordering practitioner's NPI must appear on the order and match PECOS enrollment.
42 CFR 410.38(d)Medical necessity in the clinical record
The medical record — not just the order — must support why the item is medically necessary for this patient.
CMS PA Documentation ChecklistLength of need documented for rental items
Since the 2020 SWO rule, length of need is no longer a required element of the order itself — but the DME MAC PA portals ask for it, and the record must support the expected duration of use. AffirmDME requires an estimated length of need (1–99 months; 99 = lifetime) on every capped-rental item so the packet never stalls on it.
42 CFR 410.38(d) — SWO elements (2020 rule); DME MAC PA request fieldsWhat usually sinks these claims
- The conservative-treatment month is asserted but not documented step by step — no Group 2 surface, no repositioning schedule, no wound measurements over time
- The practitioner's assessment predates the start of therapy by more than a month
- Monthly recertification statements stop after the first rental month
- A contraindication is present in the record (pulmonary disease, wet soaks, an unsupported floor) and was not addressed
- KX billed when a criterion was not met

Does E0194 require prior authorization?
From October 28, 2026, yes, nationwide, for dates of delivery on or after that day. Group 2 support surfaces have required it since 2019.
What has to happen before an air-fluidized bed is covered?
A month of conservative treatment that includes a Group 2 support surface and did not progress toward healing, then a comprehensive practitioner assessment within the month before the bed starts. The LCD lists the components of that program; every one should appear in the record.
What continued-coverage documentation is needed?
A monthly statement from the treating practitioner giving the ulcer size and healing status, what else in the care plan is being changed if it is not healing, and that continued use is reasonable and necessary. The supplier keeps these on file and bills KX only when they support continued need.
