K0005 Ultralightweight manual wheelchair base (under 30 lb, adjustable rear axle)Medicare Ultralightweight Manual Wheelchair Requirements (K0005)
Citations verified against the CMS Medicare Coverage Database · Reviewed September 2026
K0005 is the manual wheelchair for people who live in one: a full-time user, or someone whose seating needs an axle, camber, or seat-angle configuration that a K0001–K0004 base cannot provide. Medicare treats it accordingly — the general mobility criteria in LCD L33788 apply, and then a specialty evaluation and a specific supplier qualification on top.
K0005 is the first manual wheelchair base to join the Required Prior Authorization List, effective October 28, 2026, and it joins the face-to-face and written-order-prior-to-delivery list on the same day. Below is the requirement set AffirmDME checks on a K0005 case.

Mobility limitation affecting MRADLs in the home (criteria A–E)
The record must document a limitation that significantly impairs mobility-related activities of daily living in the home, that a cane or walker does not resolve, that the home supports wheelchair use, that the chair will significantly improve participation and be used regularly, and that the beneficiary is willing to use it.
LCD L33788 — manual wheelchair criteria A–EFull-time manual wheelchair use, or a need for individualized fitting
K0005 specifically requires that the beneficiary is a full-time manual wheelchair user, or requires individualized fitting and adjustments — axle configuration, wheel camber, seat and back angles — that a K0001–K0004 base cannot accommodate.
LCD L33788 — K0005 coverage criteriaSpecialty evaluation by a licensed/certified medical professional
A PT, OT, or physician with training in rehabilitation wheelchair evaluations must document the medical necessity for the chair and its features, and must have no financial relationship with the supplier. The evaluation should describe the beneficiary's routine activities and the K0005 features needed compared with a K0004.
Policy Article A52497 — LCMP specialty evaluationDiagnosis codes support the mobility limitation
The ICD-10 codes on the claim must correspond to the documented condition behind the limitation.
LCD L33788 — diagnosis supportFace-to-face encounter documented (required from October 28, 2026)
K0005 (with K0002–K0007 and E1161) joins CMS's Required Face-to-Face Encounter and Written Order Prior to Delivery List on October 28, 2026. From that date the chart notes must document an in-person (or permitted telehealth) evaluation for the item, and the completed order must reach you before delivery.
42 CFR 410.38(d)(2) — F2F/WOPD list, effective 10/28/2026Face-to-face within 6 months of the order
The qualifying encounter must have occurred within the 6 months before the order date.
42 CFR 410.38(d)(2)Standard Written Order (SWO) on file
A complete written order must exist before the claim is submitted — patient name, item description, quantity, ordering practitioner, and signature.
42 CFR 410.38(d)Order date and practitioner signature
The SWO must be dated and signed by the treating practitioner. Undated or unsigned orders are treated as no order at all.
42 CFR 410.38(d)Practitioner NPI on the order
The ordering practitioner's NPI must appear on the order and match PECOS enrollment.
42 CFR 410.38(d)Medical necessity in the clinical record
The medical record — not just the order — must support why the item is medically necessary for this patient.
CMS PA Documentation ChecklistLength of need documented for rental items
Since the 2020 SWO rule, length of need is no longer a required element of the order itself — but the DME MAC PA portals ask for it, and the record must support the expected duration of use. AffirmDME requires an estimated length of need (1–99 months; 99 = lifetime) on every capped-rental item so the packet never stalls on it.
42 CFR 410.38(d) — SWO elements (2020 rule); DME MAC PA request fieldsWhat usually sinks these claims
- The home assessment (layout, surfaces, doorways) is missing — criterion C is the most common finding in Noridian's manual wheelchair reviews
- The record describes the limitation outside the home, not in it
- No specialty evaluation, or one performed by a clinician with a financial relationship to the supplier
- The K0005-specific need (full-time use or fitting features a K0004 cannot provide) is never stated
- No Standard Written Order on file

Does a K0005 require prior authorization?
From October 28, 2026, yes, nationwide, for dates of delivery on or after that day. It is the only manual wheelchair base on the Required PA List; power wheelchairs and scooters have required it for years.
Does the PA request include the cushion and accessories?
No. CMS's notice states the prior authorization is for the wheelchair base only and does not include a review of options, accessories, seating, or positioning items. Those still need their own documentation for the claim.
What does the specialty evaluation have to say?
Why this beneficiary needs a K0005 rather than a K0004 — the fitting features required, the activities the chair is used for, and whether the beneficiary is independent in it — written by a PT, OT, or physician with wheelchair-evaluation experience and no financial tie to the supplier. Noridian expects an attestation of that independence.
What is the ATP requirement?
The LCD requires the chair to be provided by a rehabilitative technology supplier that employs a RESNA-certified Assistive Technology Professional who specializes in wheelchairs and is directly, in person, involved in selecting the chair for the beneficiary.
