L0456 TLSO — L0456 custom fitted, L0457 off-the-shelf, L0486 custom fabricated

Medicare TLSO Documentation Requirements (L0456, L0457, L0486)

Citations verified against the CMS Medicare Coverage Database · Reviewed September 2026

Spinal orthoses have one of the highest improper-payment rates in DME: CMS's 2024 review data put lumbar-sacral orthoses at over half of dollars paid in error, almost entirely for insufficient or missing documentation rather than medical necessity. The coverage rule in LCD L33790 is short; the documentation trail around it is where claims fail.

Three TLSO codes join the Required Prior Authorization List on October 28, 2026: L0456 (custom fitted), L0457 (off-the-shelf) and L0486 (custom fabricated). The requirement set below is what AffirmDME checks on all three, with the code-specific fitting and fabrication items called out.

How this gets reviewed: For dates of delivery on or after October 28, 2026, these codes need an affirmed prior-authorization decision before delivery, nationwide; requests are accepted from October 14. They also join the face-to-face and written-order-prior-to-delivery list on the same date. Standard review is up to five business days. Historically, the top denials in this category are technical — a same-or-similar orthosis within five years, or no response to the ADR — so the packet has to be complete and the history checked before the request goes in.

Required

A qualifying indication in the record

The orthosis must be ordered to reduce pain by restricting trunk mobility, to facilitate healing after a spinal injury or spinal surgery, or to support weak spinal muscles or a deformed spine — and the chart notes must say which.

LCD L33790 — spinal orthosis coverage criteria
Required

The record supports the type of orthosis ordered

The treating practitioner's record must justify prefabricated versus custom fabricated, and the supplier must bill the code that reflects both the orthosis type and the level of fitting actually performed.

Required

L0456 only: custom-fitting modifications documented

Custom fitted means more than minimal self-adjustment — trimming, bending, molding or otherwise modifying the item, by a certified orthotist or someone with equivalent training. The supplier record must describe why the modifications were necessary and what was done at fitting. If only straps and closures were adjusted, the correct code is L0457.

Required

L0486 only: custom fabrication documented

The practitioner's record must support why a custom-fabricated orthosis is necessary rather than a prefabricated one, and the orthotist's record must document the impression or measurements, the positive model (carved, modified, or CAD-CAM), and that the orthosis was individually fabricated over it.

Diagnosis codes present

The ICD-10 code(s) supporting the indication must be on the claim and in the record.

Face-to-face encounter documented (required from October 28, 2026)

L0456/L0457/L0486 joins CMS's Required Face-to-Face Encounter and Written Order Prior to Delivery List on October 28, 2026. From that date the chart notes must document an in-person (or permitted telehealth) evaluation for the item, and the completed order must reach you before delivery.

42 CFR 410.38(d)(2) — F2F/WOPD list, effective 10/28/2026
Required

Face-to-face within 6 months of the order

The qualifying encounter must have occurred within the 6 months before the order date.

42 CFR 410.38(d)(2)
Required

Standard Written Order (SWO) on file

A complete written order must exist before the claim is submitted — patient name, item description, quantity, ordering practitioner, and signature.

42 CFR 410.38(d)
Required

Order date and practitioner signature

The SWO must be dated and signed by the treating practitioner. Undated or unsigned orders are treated as no order at all.

42 CFR 410.38(d)
Required

Practitioner NPI on the order

The ordering practitioner's NPI must appear on the order and match PECOS enrollment.

42 CFR 410.38(d)
Required

Medical necessity in the clinical record

The medical record — not just the order — must support why the item is medically necessary for this patient.

CMS PA Documentation Checklist
Required

Length of need documented for rental items

Since the 2020 SWO rule, length of need is no longer a required element of the order itself — but the DME MAC PA portals ask for it, and the record must support the expected duration of use. AffirmDME requires an estimated length of need (1–99 months; 99 = lifetime) on every capped-rental item so the packet never stalls on it.

FAQ

Which spinal orthosis codes require prior authorization?

Before October 28, 2026: L0631, L0637, L0639, L0648, L0650 and L0651 (LSOs). From that date the three TLSO codes L0456, L0457 and L0486 are added, nationwide. Use the PA List lookup on this site to check any code.

What documentation makes a TLSO 'custom fitted'?

A record of substantial modification at delivery — trimming, bending, heat-molding — performed by a certified orthotist or equivalently trained individual, with a description of why it was needed and what was done. Adjusting straps and closures is minimal self-adjustment and belongs to the off-the-shelf code.

Is a PDAC coding verification required?

Yes for L0456 and L0457 — the product must appear on the PDAC Product Classification List for the code billed. Custom-fabricated L0486 needs PDAC verification only when fabricated by a manufacturer or central fabrication facility.

What is the review timeframe?

The DME MACs target five business days for a standard initial request and for resubmissions, and two business days for an expedited request. An affirmed decision comes with a UTN that goes on the claim.
