All DME Applies to every DMEPOS claim

Medicare DME Documentation Requirements: the checklist every claim needs

Citations verified against the CMS Medicare Coverage Database · Reviewed September 2026

Before any equipment-specific criteria apply, Medicare DME claims have to clear the same baseline: a complete Standard Written Order, a documented face-to-face encounter (required for items on CMS's face-to-face list — which includes every category in these guides), and a clinical record that supports medical necessity. Most denials aren't exotic — they're one of these basics missing or dated wrong.

This is the exact baseline checklist AffirmDME evaluates on every case, drawn from 42 CFR 410.38(d) and CMS's prior-authorization documentation guidance.

How this gets reviewed: These requirements apply to all DMEPOS claims regardless of whether the specific HCPCS code is on CMS's required prior-authorization list — they're enforced through prepayment review, Additional Documentation Requests (ADRs), and post-payment audits.

The requirements, with citations

Required

Standard Written Order (SWO) on file

A complete written order must exist before the claim is submitted — patient name, item description, quantity, ordering practitioner, and signature.

42 CFR 410.38(d)
Required

Order date and practitioner signature

The SWO must be dated and signed by the treating practitioner. Undated or unsigned orders are treated as no order at all.

42 CFR 410.38(d)
Required

Practitioner NPI on the order

The ordering practitioner's NPI must appear on the order and match PECOS enrollment.

42 CFR 410.38(d)
Required

Face-to-face encounter documented

Chart notes must document an in-person (or permitted telehealth) encounter where the practitioner evaluated the patient for the item.

42 CFR 410.38(d)(2)
Required

Face-to-face within 6 months of the order

The qualifying encounter must have occurred within the 6 months before the order date.

42 CFR 410.38(d)(2)
Required

Medical necessity in the clinical record

The medical record — not just the order — must support why the item is medically necessary for this patient.

Required

Length of need documented for rental items

Since the 2020 SWO rule, length of need is no longer a required element of the order itself — but the DME MAC PA portals ask for it, and the record must support the expected duration of use. AffirmDME requires an estimated length of need (1–99 months; 99 = lifetime) on every capped-rental item so the packet never stalls on it.

Required

Patient demographics complete

Patient sex and a complete mailing address are required on the PA request / claim (CMS-1500 fields 3 and 5).

Required

Prior UTN on resubmissions and appeals

Resubmitting or appealing? The prior Unique Tracking Number must accompany the request so the MAC can link it to the original decision.

FAQ

What is a Standard Written Order (SWO)?

The written order 42 CFR 410.38(d) requires for DMEPOS items: patient name, item description, quantity, the ordering practitioner's name and NPI, order date, and the practitioner's signature. Since 2020 it replaced the older detailed written order formats.

Does every DME item need prior authorization?

No. Only items on CMS's Required Prior Authorization List (mainly power mobility devices and certain other items) need a PA decision before delivery. Everything else is still reviewed against the same documentation requirements — just after submission, via prepayment review or ADRs.

What happens if documentation is missing?

The claim is denied or an ADR letter is issued asking you to produce the records. Reworking a single denial costs suppliers roughly $118 on industry estimates — catching the gap before submission is far cheaper.
