Medicare Wheelchair & Power Mobility Prior Authorization (E1161)

Citations verified against the CMS Medicare Coverage Database · Reviewed September 2026

Mobility devices carry the heaviest process in DME: power mobility devices sit on CMS's required prior-authorization list, and complex manual chairs like tilt-in-space (E1161) are judged against the same mobility framework — a documented limitation of mobility inside the home that the device resolves.

AffirmDME evaluates these checks on every mobility case, from the wheelchair LCDs' clinical criteria through the MAC guidance layered on top.

How this gets reviewed: Power mobility devices require an affirmative prior-authorization decision before delivery, and E1161 joins the face-to-face and written-order-prior-to-delivery list on October 28, 2026. The mobility examination and home-use justification are the core of the packet, and the exam has to be recent: within the 6 months before the order.

The requirements, with citations

Every item below is a check AffirmDME runs automatically — this list mirrors the production rules engine, not a paraphrase of the regulations.

Required

Mobility limitation in the home documented

The record must document a mobility limitation that impairs the patient's ability to participate in mobility-related activities of daily living *in the home* — Medicare's mobility analysis is in-home, not community.

LCD L33788 — manual wheelchair mobility criteria
Required

Face-to-face mobility exam within 6 months of the order

The in-person mobility evaluation must fall within the 6 months before the order. The older PMD-specific 45-day window was removed by the 2020 order rule; 42 CFR 410.38(d)(2) now sets one timing rule for every item on the face-to-face list, power and manual alike.

42 CFR 410.38(d)(2) — face-to-face timing
Required

Diagnosis codes supporting the mobility limitation

The claim's diagnosis codes must align with the documented limitation.

Required

LCMP (PT/OT) specialty evaluation

Tilt-in-space chairs require a specialty evaluation by a licensed/certified medical professional — a PT, OT, or physician with rehab wheelchair training — who has no financial relationship with the supplier. A written report must be available on request.

Policy Article A52497 — Manual Wheelchair Bases
Required

Standard Written Order (SWO) on file

A complete written order must exist before the claim is submitted — patient name, item description, quantity, ordering practitioner, and signature.

42 CFR 410.38(d)
Required

Order date and practitioner signature

The SWO must be dated and signed by the treating practitioner. Undated or unsigned orders are treated as no order at all.

42 CFR 410.38(d)
Required

Practitioner NPI on the order

The ordering practitioner's NPI must appear on the order and match PECOS enrollment.

42 CFR 410.38(d)
Required

Face-to-face encounter documented

Chart notes must document an in-person (or permitted telehealth) encounter where the practitioner evaluated the patient for the item.

42 CFR 410.38(d)(2)
Required

Medical necessity in the clinical record

The medical record — not just the order — must support why the item is medically necessary for this patient.

CMS PA Documentation Checklist
Required

Length of need documented for rental items

Since the 2020 SWO rule, length of need is no longer a required element of the order itself — but the DME MAC PA portals ask for it, and the record must support the expected duration of use. AffirmDME requires an estimated length of need (1–99 months; 99 = lifetime) on every capped-rental item so the packet never stalls on it.

FAQ

Which mobility devices require Medicare prior authorization?

CMS's Required Prior Authorization List covers power mobility devices nationwide. Complex manual chairs are evaluated under the same LCD mobility framework, and several MACs apply additional documentation expectations — check your jurisdiction.

Is the mobility exam window still 45 days?

No. The 45-day rule was specific to power mobility devices and was removed when CMS consolidated the order rules in 2020. Today 42 CFR 410.38(d)(2) requires the face-to-face encounter within the 6 months before the order for every item on the face-to-face list, and the power mobility LCD follows it. Older exams still fail — 6 months is the line, and it is the most common timing failure on mobility claims.

Do requirements differ by MAC?

Yes. The LCDs and their policy articles set the national baseline, and MACs publish additional jurisdiction guidance on top. AffirmDME scopes its checklist to your case's jurisdiction and MAC automatically.
