Adding locations (PTAN/NPI)
A location in AffirmDME represents a billing entity: a physical site with its own PTAN and NPI. Every case is created under a location, which drives:
- MAC routing — the location’s jurisdiction and DME MAC (Noridian or CGS) pre-fill the case
- Billing — subscription plans are priced per location, and included-case allowances count per location
- Outreach — the location’s fax number is the reply-to for prescriber record requests
Adding one
Locations → Add location: name, address, PTAN (5–10 letters/digits), NPI, jurisdiction (A–D), MAC, phone, and a fax number — required before that location can send prescriber outreach faxes.
Probationary prior authorization (newly enrolled locations)
From October 15, 2026, a location that is newly enrolled in Medicare, or that completes a change of ownership on or after that date, must obtain a prior-authorization decision on 40 additional orthotic and bone-growth-stimulator codes for one year from its first claim for one of them. If that applies, enter the date the year ends in Probationary PA ends. AffirmDME then flags those codes on the case form and in the rules library for that location until the date passes. Leave the field blank for established locations. Background: the probationary PA guide.
Things to know
- On per-location plans, your subscription quantity follows your location count — adding a location adds a per-location fee at your tier’s rate
- Cases can’t move between locations; pick the right one at creation
- Multi-state operations: each location carries its own jurisdiction, so cases route to the right MAC automatically
